ICSI Treatment : Alternative Infertility Treatments Mumbai India

What is ICSI?

ICSI or intracytoplasmic sperm injection is used as a part of IVF treatment. For male infertility where there is a defect in sperm quality, intracytoplasmic sperm injection (ICSI) may be used, where a sperm cell is injected directly into the egg cell. This is used when sperm has difficulty penetrating the egg, and in these cases the partner's or a donor's sperm may be used.

Why ICSI ?

ICSI has been found to increase the success rates of IVF!
ICSI is also used when sperm numbers are very low!
Where there is a defect in sperm quality !
When there is poor sperm movement !
When the sperm is abnormally shaped !

When is ICSI done?

ICSI is done if regular IVF treatment cycles have not achieved fertilization.

It is done in cases of retrograde ejaculation where the sperm is retrieved from the man’s urine.

Sperm retrieval through TESE (Testicular Sperm Extraction) which is used when sperm is produced but not found in his ejaculate.

What is the procedure of ICSI?

ICSI is generally done as a part of IVF. Since ICSI is done in the lab, your IVF treatment won't seem much different than an IVF treatment without ICSI.

With regular In vitro fertilisation (IVF) process an egg is fertilized by sperm outside the body: in vitro. IVF is a major treatment for infertility when other methods of assisted reproductive technology have failed.

The process involves monitoring and stimulating a woman's ovulatory process, removing ovum or ova (egg or eggs) from the woman's ovaries and letting sperm fertilize them in a fluid medium in a laboratory.

If fertilization takes place, and the embryos are healthy, an embryo or two will be transferred to your uterus, via a catheter placed through the cervix, two to five days after the retrieval

Once the eggs are retrieved, an embryologist will place the eggs in a special culture, and using a microscope and tiny needle, a single sperm will be injected into an egg. This will be done for each egg retrieved.

How much does ICSI cost?

On top of the general IVF cost, ICSI typically costs between $ 1000 and $1500 which on average costs $12000 to $15000.

Is the baby safe in ICSI?
ICSI treatment carries a slightly increased risk of birth defects than normal pregnancy which carries a risk of 1.5 to 3%. Commonly found birth defects in ICSI includes Beckwith-Wiedemann syndrome, Angelman syndrome, hypospadias, and sex chromosome abnormalities.

There is some increased risk of a male baby having fertility problems in the future. This is because male infertility may be passed on genetically.

Chances of success rates by couples using ICSI with IVF compared to regular IVF is same once the fertilization happens. Even though ICSI procedure fertilizes 50 to 80% eggs just because a sperm is injected into an egg it does not guarantee fertilization will happen surely or sometimes the embryo may stop growing.

How to boost fertility with laproscopy?

 Reproductive Endoscopic surgery Mumbai India

Laproscopic Surgery for Infertility in Mumbai

A underlying desire for every female is to travel the essence of motherhood offering a beginning to a toddler the marvelous creation, she cuddles that motherhood. But it is a sour fact that no longer each couple is blessed to have and possess children. Most of the days it’s going to move all the way down to the fertility concerns with the woman. So first we have to understand what girl infertility is or in other terms the common lack of ability of a female to endure a child.

A female’s infertility reason could be detected through total physique examination. There can be various causes attributed to infertility and one of these is termed as endometriosis which is a gynecological condition in which cells from the lining of the uterus appears and grow outside the uterine cavity which lines the abdominal cavity. Endometriosis has a significant social and psychological impact.

Easy to be pregnant after a laproscopy, How?

Laparoscopy is a surgical procedure that has two separate functions, diagnosis and treat disease. It is just a routine element of pregnancy work-up as some irregularities including endometriosis and tubal problem can just only be assessed through this process.

A diagnostic laparoscopy is used to determine the severity of a number of conditions within the body and to figure out why a person is experiencing symptoms.

Diagnostic laparoscopy is just a process that’s done to check out the tissues and organs inside the stomach. It’s an important part of infertility work-up as some problems including endometriosis and tubal harm can’t be trusted and accordingly evaluated by every other method of analysis. Along with analysis, this gives the chance to surgeon to deal with a few of the problems in the same sitting in the place of subject the in-patient through still another anesthesia and surgical treatment.

In fertility treatment, a laparoscopy is used to examine the pelvic region. In many medical cases now, it is possible to use laparoscopy instead of a “bikini cut” surgery which involves a much larger incision. In Laparoscopy the cut is within the belly-button (umbilicus) and small (key-hole). Extra 2 to 3 little (0.5 to 1cm) incisions might be made based on need. In this surgery, a telescope is inserted often through the umbilicus and the peritoneal cavity and the areas within it are inspected directly. The amplified pictures are shown on monitors. Many issues for example irregularities using the adhesions, uterine fibroids, endometriosis, and pipes could be identified.

The technique is often used to evaluate for cysts, fibroids, as well as to check for scar tissue, endometriosis or other abnormalities. In those cases where it is required, it is possible to collect tissue samples through the tube being used for a laparoscopic procedure. Additionally, often at the time of a laparoscopy the fallopian tubes are evaluated with “blue dye”. The dye is injected thru the uterus up to the fallopian tubes and hopefully if the tubes are open the dye will end up coming out the end of the tubes into the abdomen.

The time taken by patients to recuperate from the surgery is much lesser as is the pain, as the incisions are much smaller than the conventional abdominal procedures. Key-hole surgery or laparoscopy can also be referred to as minimally-invasive surgery/band-aid surgery.

A laparoscopic treatment will carefully remove these tissue growths (endometriosis) or adhesions in order to provide the pelvic organs a better chance of being able to implant an embryo and achieve pregnancy. While not without risk, the laparoscopy is a minimally invasive option that may provide exceptional pain relief, in addition to improving fertility.

The benefits of laparoscopic surgery include less trauma and incision scarring when compared to open abdominal surgery, as tiny incisions are used for the “portals”.

Remember, whilst taking into account a girl infertility treatment, make use of all that technology has to provide that may make a contribution for your well-being.

If pregnancy has not been achieved after 3-6 months of treatment, ideally laparoscopy should be done. The skill of surgeon is very important for good result. The equipments also play important role.

Female Infertility treatment. How can your doctor help?

Female Infertility treatment. How can your doctor help?

What is Infertility

Infertility means not being able to get pregnant after one year of trying. If a woman is 35 or older. Women who can get pregnant but are unable to stay pregnant may also be infertile.

Pregnancy is the result of a process that has many steps. To get pregnant:
  • A woman must release an egg from one of her ovaries (ovulation).
  • The egg must go through a Fallopian tube toward the uterus (womb).
  • A man's sperm must join with (fertilize) the egg along the way.
  • The fertilized egg must attach to the inside of the uterus (implantation).
If there are any problems with any of the above steps Infertility can happen.

Female Infertility and its causes

Most cases of female infertility are caused by ovulation problems as without ovulation, there are no eggs to be fertilized. Ovulation problems are often caused by polycystic ovarian syndrome (PCOS) which is a hormone imbalance problem Some signs that a woman is not ovulating normally include irregular or absent menstrual periods.PCOS is the most common cause of female infertility. Primary ovarian insufficiency (POI) is another cause of ovulation problems. POI occurs when a woman's ovaries stop working normally before she is 40.

Female infertility can also vary widely by geographic location around the world as it is usually more or less a combination of nature and nurture.

Other known causes of fertility problems in women include:

  • Blocked Fallopian tubes
  • Physical problems with the uterus
  • Uterine fibroids, which are non-cancerous clumps of tissue and muscle on the walls of the uterus.
Additional causes to increase a woman's risk of infertility?

Many things can change a woman's ability to have a baby. These include:
  • Age
  • Stress
  • Poor diet
  • Athletic training
  • Being overweight or underweight
  • Smoking
  • Excess alcohol use
  • Sexually transmitted infections (STIs)
Doctors Diagnosis : Infertility checkup
Doctors generally do infertility checkup which involves a physical exam.

In women, the first step is to find out if she is ovulating each month. There are a few ways to do this. A woman can track her ovulation at home by:
  • Writing down changes in her morning body temperature for several months
  • Writing down how her cervical mucus looks for several months
  • Using a home ovulation test kit (available at drug or grocery stores)
Doctors can also check ovulation with blood tests. Other common tests are laproscopy and hysterosalpingography.

Infertility treatment

Infertility treatment is usually a combination of drugs and surgery. Artificial insemination or IUI with assisted reproductive technology (ART) is also used to treat based on the tests done.

InVitro Fertilization today in India

What is Invitro Fertilization and who can go for it

In vitro fertilisation (IVF) is a process by which eggs are fertilised by sperms outside the body: in vitro in Latin means glass, because earlier biological experiments involving tissue cultivation outside the living organism was carried out in glass containers. That is how babies born with IVF procedures are also called test tube babies. IVF is a major treatment for infertility when other methods of assisted reproductive technology have failed. The process involves monitoring and stimulating a woman's ovulatory process, removing a healthy ovum or ova (egg or eggs) from the woman's ovaries and letting sperm fertilise them in a fluid medium in a laboratory. The fertilised egg (zygote) cultured for 2–6 days in a growth medium and is then transferred to the patient's uterus with the intention of establishing a successful pregnancy.

Females who have problems in the fallopian tube and men who have defect in sperm quality are suggested to go for the IVF treatment.

The method follows:

in vitro fertilisation could be performed by collecting the contents from a woman's fallopian tubes or uterus after natural ovulation, mixing it with semen, and reinserting into the uterus. But additional techniques are used for successful pregnancy like ovarian hyperstimulation to retrieve multiple eggs through ultrasound guided transvaginal egg retrieval, egg and sperm preparation, culture and resultant embryo transfer into the uterus. IVF can be performed by collecting a naturally selected egg from woman’s natural menstrual cycle without the use of any drugs. The next step-up method which is called modified natural cycle IVF, medication is used for 2–4 days during woman’s natural cycle to avoid spontaneous ovulation and to make the treatment more successful. Semen is prepared for fertilisation by removing inactive cells and seminal fluid in a process called sperm washing and if it is provided by a sperm donor, it is usually prepared for treatment before being frozen.

The sperm and the egg are incubated together at a ratio of about 75,000:1 in a culture media as per studies in 2013 that a duration of 1 to 4 hours results in higher pregnancy rates than 16 ot 24 . intracytoplasmic sperm injection (ICSI) process is undertaken where a single sperm is injected into the egg in situations where it is low sperm count.

IVF success rates are the percentage of all IVF procedures which result in a favorable outcome.

About 10-12 percent of Indians have trouble conceiving a child. But several myths keep couples from seeking help

Some Myths vs Facts about IVF and Pregnancy: Courtesy Dr Firuza Parekh (The Complete Guide to Becoming Pregnant)

Myth
Eating spicy food causes ovulation, low sperm count, miscarriage, and premature delivery.
Fact
Eating spicy food only occasionally disturbs the gastrointestinal system and sometimes causes heartburn.
Myth
Seven to eight days of abstinence are required before a semen analysis, or before giving a sample for IVF/ ICSI/ sperm freezing.
Fact
Increased days of abstinence can, in fact, decrease the sperm motility. Two or four days of abstinence are adequate for a good semen sample.
Myth
Masturbation causes impotency and low sperm count.
Fact
Masturbation should not be associated with guilt. It does not impact fertility.
Myth
The chances of pregnancy are reduced if the ejaculated semen comes out of the vagina after intercourse.
Fact
Ejaculated semen liquefies after a few minutes in the vagina. Some seminal fluid is bound to come out. This does not decrease the chances of pregnancy.
Myth
The sexual position determines the gender of the baby.
Fact
No sexual position has any effect on the gender of the baby. There may be other compelling reasons though, for some sexual variation.
Myth
Having sex daily increases the chances of pregnancy.
Fact
Daily sex does not increase the chances of pregnancy and may, in fact, decrease the sperm count on the day of ovulation. In men with low sperm counts, this may be detrimental to the odds of pregnancy.
Myth
PCOS (polycystic ovary syn drome) goes away after having a baby.
Fact
PCOS is a condition that has a genetic basis. It does not go away by itself. Treatment needs to be continued.

Infertility Treatment for Men and Women in Mumbai India

"Of all nature's gifts to the human race, what is sweeter to a man than his children?" Marcus Tullius Cicero

Infertility Solution Provider in India MumbaiInfertility, whether male or female, is defined as the
inability of a couple to achieve conception or bring a
pregnancy to term after a year or more of regular,
unprotected sexual intercourse. For women
over age 35, this condition is diagnosed after 6
months of an inability to conceive.
According to WHO, between 2 and 10% of
couples worldwide are unable to conceive a child i.e. primary infertility and a further of 10-25% experience secondary Infertility i.e. are unable to conceive a second or subsequent child. Among these couples, infertility is exclusively a problem in the female in about 30-40% of cases, exclusively in the men in about 30-40% of cases and unexplained in 5-10% of couples. WHO believes that around 60-80 million couples in the world are infertile. The normal reproduction process requires interaction between the female and male reproductive tracts.
The woman ovulates and releases an egg from her ovaries to travel through the fallopian tube to her uterus (womb). The male produces sperm. Both egg and sperm normally meet in the woman's fallopian tube, where fertilization occurs. The embryo then implants in the uterus for further development.
Causes of Infertility
Causes in either SexGynaecologist in Mumbai India
  1. Genetic factors - A Robertsonian translocation in either partner may cause recurrent spontaneous abortions or complete in fertility.
  2. General factors like diabetes mellitus, age
  3. Hyperprolactinemia
  4. Hypopituitarism
  5. Thyroid disorders
  6. Environmental factors - Toxins such as glues, volatile organic solvents or silicones, physical agents, chemical dusts, and pesticides. Tobacco smokers are 60% more likely to be infertile than non-smokers.
Specific Female Causes
Factors that can affect a woman's chances of conceiving include being over- or underweight, or her age as female fertility declines sharply after the age of 35.

Surrogate treatment in Mumbai India
ICSI
Medical treatment of infertility generally involves the use of fertility medication, medical device, surgery, or a combination of the following. If the sperm are of good quality and the mechanics of the woman's reproductive structures are good (patent fallopian tubes, no adhesions or scarring), physicians may start by prescribing a course of ovarian stimulating medication.
The physician may also suggest intrauterine insemination (IUI), in which the doctor introduces sperm into the uterus during ovulation, via a catheter. In these methods, fertilization occurs inside the body.
If conservative medical treatments fail to achieve a full term pregnancy, the physician may suggest the patient to undergo in vitro fertilization (IVF). IVF and related techniques (ICSI, ZIFT, GIFT) are called assisted reproductive technology (ART) techniques.

Sexology treatment in Mumbai India
In Vitro Fertilization
In Vitro Fertilization
ART techniques generally start with stimulating the ovaries to increase egg production. After stimulation, the physician surgically extracts one or more eggs from the ovary, and unites them with sperm in a laboratory setting, with the intent of producing one or more embryos. Fertilization takes place outside the body, and the fertilized egg is reinserted into the woman's reproductive tract, in a procedure called embryo transfer.

Some types of infertility treatment can cause complications including:
  • Side effects of medication
  • Increased risk of ectopic pregnancy
  • Multiple pregnancy
  • Stress
