Difficulty in getting pregnant – PCOS can be a cause

What is PCOS (Polycystic ovary syndrome)
 PCOS is a hormonal imbalance in women that is considered to be one of the leading causes of female infertility .It can cause problems with your periods and also cause unwanted changes in the way you look. People can get diabetes or heart problem if this is not treated on time. The symptoms of PCOS can start from your teens where the hormones get out of balance and as these hormones are mainly the chemicals which triggers different processes including growth and energy production. If one hormone does not function properly the release of another hormone gets affected.

For eg:
When the sex hormones get out of balance the ovaries which normally make a tiny amount of male sex hormones or androgens when affected with PCOS they start making slightly more androgens which can cause you to stop ovulating, get acne, and grow extra facial and body hair.

Similiarly sometime the body may have a problem using insulin, called insulin resistance. When the body doesn't use insulin well, blood sugar levels go up. Over time, this increases your chance of getting diabetes.

Causes for PCOS
There may be one or more causes for the hormone level changes. The main cause can be heriditary where if other women in your family has irregular periods, or diabetes or diagnosed with PCOS your chances are higher. It can be both from mother's or father's side.

Symptoms of PCOS

The common symptoms are:


Acne.
Weight gain/trouble losing weight.
Extra hair on the face and body.
Thinning hair on the scalp.
Irregular periods(maybe lesser than 9 periods in a year/no periods/ heavy bleeding.
Fertility problems.
Depression.

Diagnosis of PCOS
  • Answer the doctor with the questions about your past health, symptoms, and menstrual cycles.
  • Do a physical exam to look for signs of PCOS, such as extra body hair and high blood pressure.
  • Check your body mass index (BMI).
  • Check your blood sugar, insulin, and other hormone levels. Hormone tests can help rule out thyroid or other gland problems that could cause similar symptoms.
  • Have a pelvic ultrasound to look for cysts on your ovaries.

How is PCOS treated?

Infertility Treatment : Which suits you?



Fertility problems have effective treatments as they are mostly medical in nature. Worldwide, more than 3 million babies have now been born through in vitro fertilization (IVF) and there are other therapies which are successful through which half of the couples have chances of getting pregnant.

Symptoms of Infertility that should not be delayed

Relaxing will not overcome infertility. Most couples should consult a doctor after a year of trying to conceive unsuccessfully.

If the woman is over age 35 and hasn't conceived after trying for six months or has an irregular menstrual cycle, it's best to see a doctor as soon as possible.

Male infertility is just as common as female infertility. So a man should be evaluated, too.

Possible causes of Male Infertility

Low sperm count
Poor sperm motility
Malformed sperm
Blocked sperm ducts

The woman is diagnosed with a problem, such as:

Diagnosing the cause

By tracking Ovulation

Ovulation tests are conducted to find out when you're ovulating (and determine the best time for sex). These detect a hormonal surge that occurs 12 to 36 hours before the ovary releases an egg.Irregular ovulation accounts for about a third of all cases If the tests never yield a positive result, consult your doctor.

Fertility Drugs

If your doctor determines you're not ovulating normally, fertility drugs can help. The most common choice is Clomid and Serophene which are relatively inexpensive and effective. Within three cycles 50% women gets pregnant and Clomid increases the chance of multiple births.
Injectable Hormones

Injections of fertility hormones are suggested if you don't get pregnant after taking clomiphene for six months. A wide range of highly effective hormonal drugs are available helping in stimulating ovulation. Like clomiphene, injectable hormones increase the chances of becoming pregnant with multiples.

Surgery for Blocked Fallopian Tubes

Scarring caused by endometriosis which is a overgrowth of tissue that lines the uterus, a history of pelvic infections, or previous surgeries can be the general causes.Mostly women have trouble getting pregnant because scar tissue prevents eggs from traveling down the fallopian tubes. Laparoscopic surgery can remove scar tissue in the reproductive tract and boost the odds of getting pregnant for some women.

Infertility Treatments That Work

Intrauterine Insemination (IUI)

IUI is a  procedure, through which the sperm is placed directly into the woman's uterus while she is ovulating. This reduces the distance the sperm must swim to reach the egg. IUI or Intrauterine insemination (IUI) is a popular option for a wide range of fertility problems. It is less expensive and less invasive than IVF, but pregnancy rates are notably lower.

IUI With Donor Sperm

IUI can be done using sperm from a donor if the patient is having low quality sperms. Most doctors recommend couples see a counselor before choosing this option, because they must be comfortable raising a child who is not biologically related to the father. IUI with donor sperm has a very high success rate in fertile women.

In Vitro Fertilization (IVF)

IVF offers hope when other infertility treatments are unsuccessful. It eliminates any barriers between egg and sperm by combining them in a lab. The growing embryos are then placed inside the uterus. Undergoing IVF can be unpleasant and expensive but success rates are growing every year.

IVF With ICSI

Applicable for men infertility, when a man's sperm count is extremely low or the sperm don't move well, a procedure called intracytoplasmic sperm injection (ICSI) can overcome this problem by inserting a single sperm directly into an egg. The resulting embryos are then transferred to the uterus through the normal IVF procedure. The majority of IVF cycles now use ICSI.

IVF and Multiples

To boost the odds of success with IVF, it's common to transfer two to four embryos at a time. But this means the woman may become pregnant with twins, triplets, or even quadruplets. Carrying multiples raises the risk of miscarriage, anemia, high blood pressure, and other complications during pregnancy. It also makes premature birth more likely. Couples undergoing IVF should discuss these issues with their fertility specialist.

IVF with Blastocyst Transfer

Blastocyst transfer  is a recent breakthrough in IVF technology. In standard IVF, embryos are transferred to the womb when they reach the two- to eight-cell stage whereas in this procedure, the embryos are allowed to grow for five days until they reach the blastocyst stage. The healthiest one or two blastocysts are chosen for transfer. This eliminates the possibility of triplets while maintaining a high success rate.

Using Donor Embryos – Not biologically related to either parent

Couples who have not had success with IVF or are looking for a less expensive option may want to consider using donor embryos. These are embryos donated by other couples who have finished the IVF process. Transferring donor embryos costs less than standard IVF or IVF with donor eggs. If successful, the procedure allows a couple to experience pregnancy. The baby will not be biologically related to either parent.

Surrogate Pregnancy – Gestational where the baby is the biological child

For women who have trouble carrying a pregnancy to term, surrogacy is an option. In traditional surrogacy, the surrogate was inseminated with the husband's sperm. But today, most couples choose gestational surrogacy. IVF is used to create embryos with the wife's eggs and husband's sperm. The embryos are transferred to the surrogate. If the procedure succeeds, the baby will be the biological child of both husband and wife.

Whether the patients are kept regularly updated about the treatment decisions
But don't base your choice solely on these rates as infertility treatment is a long-term process, and you want to feel comfortable with your clinic.

Here is a quick way for solving female infertility through Tubal Cannulation Mumbai

Ratio of 1 in 4 women have difficulty getting pregnant due to blockage in fallopian tubes and Tubal cannulation is a procedure to help clear this blockage which is a common cause of female infertility.





Tubal cannulation may be done immediately after a procedure called hysterosalpingography or hysterosalpingogram. During this procedure, dye is first flushed through the catheter to identify and locate a fallopian tube blockage.

The doctor inserts a tube called a catheter that is guided over a wire. Ultrasound or real-time moving X-rays of the fallopian tubes may be used to guide your doctor to the correct area. The blocked area in the fallopian tube is opened up using a balloon on the catheter or with the wire.

Tubal cannulation can be done with or without anesthesia where you are given a mild sedative to relax and the procedure is less invasive than fallopian tube surgery and helps the doctor to better understand why the blockage occurred.

Usually it is an outpatient procedure which does not need a hospital stay and you can likely go home the same day. You can avoid more expensive and invasive surgical procedures.

Who should go for Tubal Cannulation ?

Once the sonography is done if the imaging test clearly shows a blockage in one or both fallopian tubes tubal cannulation is suggested. The cannulation procedure is most successful when the blockage occurs in the part of the tube closest to the womb (uterus), which is called a proximal tubal obstruction. It can be used for mid-tubal blockage also.

Some doctors recommend that women should consider tubal cannulation before having more expensive fertility procedures, such as in vitro fertilization (IVF).

Who should avoid Tubal Cannulation?

Tubal cannulation is not recommended for:
  • Patients having Genital tuberculosis and certain other infections
  • Who have undergone fallopian tube surgery before
  • If there is extensive scarring or damage in the fallopian tubes
  • Severe blockage that is difficult for a catheter to pass through
  • Distal blockage (in an area of the fallopian tube that is the farthest from the uterus)
Tubal cannulation may fail or may not work as well if you have:

  • A blockage in a part of the fallopian tube far away from the uterus
  • Certain blockages in the narrowest part of the fallopian tube, called the isthmus
  • Inflammatory condition of the fallopian tubes
  • Severe tubal disease or scarring
  • Tuberculosis
Risks of Tubal Cannulation

Tubal cannulation should only be done by a doctor who is well-trained in the procedure. Risks include: Failure to restore fallopian tube function or tearing or perforaton of the fallopian tube wall. Chances of a life threatening infection called peritonitis which occurs in the tissue covering the abdominal organs.

What can be expected after Tubal Cannulation

It is important to remember that successfully reopening the fallopian tubes does not always help a woman become pregnant as pregnancy rates depend on the:

  • Specific procedure performed
  • Location of the blockage
  • Cause of the blockage
Tubal cannulation helps restore fertility in many, but not all, women.

Women with severe fallopian tube disease who are not good candidates for tubal cannulation may wish to consider IVF and embryo transfer.
