What is new to increase success rate in IVF? Simple scratch could double success of IVF treatment

Researchers have reported that gently scratching the lining of the uterus before IVF treatment shown to increase the clinical pregnancy rate of women undergoing IVF. This procedure is known as endometrial scratching.

It is thought that scratching the womb lining stimulates growth factors and repairs the mechanism that allow the fertilized egg to embed more easily. Endometrium scratching is a short OPD procedure which hardly takes 15 minutes in clinic and can be carried out by trained nurses using simple equipment already in use.

Retrograde Ejaculation And Male Fertility

Retrograde ejaculation is a condition that prevents a man from ejaculating semen during sexual intercourse. This can, unsurprisingly, lead to infertility.

What are the symptoms of retrograde ejaculation, and what are the possible treatments?

What is Retrograde Ejaculation?

During normal male ejaculation, sperm are released from the testicles. They travel up through the vas deferens, small tubes leading from the scrotum to the penis. The sperm mix with seminal fluid, and the mixture is then ejaculated via the urethra after the bladder sphincter closes. Retrograde ejaculation is a situation in which the bladder sphincter does not contract properly. The bladder then becomes the easier exit for the sperm, instead of the urethra.

Men who suffer from retrograde ejaculation can still have an orgasm, but with very little to no semen coming out. That is why retrograde ejaculation is also sometimes referred to as a dry orgasm. Infertility is an obvious expected consequence of this problem. Retrograde ejaculation is not medically dangerous. The semen that ends up in the bladder simply comes out with the man’s urine, instead. Cloudy urine is one symptom that may give the problem away.

Retrograde ejaculation can be caused by a variety of things. They include certain medications (especially hypertension meds), health problems (including diabetes and multiple sclerosis), and even surgery. Any man who notices that ejaculation doesn't produce semen may like to see a doctor about this in any circumstance, but especially if they would like to get their partner pregnant.

Retrograde Ejaculation and Infertility

Some men who suffer from retrograde ejaculation will still have some semen coming out with their ejaculations. In this case, there is certainly a reduced chance that the man will get his partner pregnant. There is less sperm in the ejaculate that reaches the female reproductive system, after all. But pregnancy may still be possible.

Retrograde ejaculation can be treated with medication in some men. Those who have the problem because of a physical injury, or as the result of a surgery, will not benefit from medical treatment. But those men who have nerve damage as the result of a health condition such as diabetes may well benefit from certain medications. The drugs that doctors sometimes use to correct retrograde ejaculation were not designed for that purpose. They are antihistamines, antidepressants, and decongestants.

Men whose retrograde ejaculation is actually caused by medication may have another possibility — stopping their drug regime for a while, during the period in which they are trying to conceive, or switching to other medications that may not cause the same problem. Needless to say, this is always something that should be done under strict medical supervision.

Cases that are caused by physical problems cannot be cured by medication. These men can still have the chance to become biological fathers with the help of artificial reproductive techniques. ICSI (intracytoplasmic sperm injection) is a form of IVF Treatment in which one sperm is directly injected into one egg. Men who do produce sperm but cannot ejaculate it can benefit from this procedure by having sperm harvested directly from the testicles. There are techniques called TESA/PESA which is very much useful in such patients. Sperm can also be recovered from the bladder, medically processed, and then used in ART techniques.

Retrograde ejaculation is only one of many causes of Male Infertility Treatment in Mumbai. If you are affected by this problem, or your partner is, you will like to know that many men who have retrograde ejaculation are indeed able to father children once they seek treatment. Do not try to conceive naturally for 12 months before seeking treatment if you suspect you suffer from retrograde ejaculation. Fertility treatments have a lower chance of success as the female partner gets older, so you will benefit from treatment as soon as possible.

What Patients want from IVF Doctors - IVF Treatments

IVF can be an emotionally taxing treatment.
Not only is it very expensive, there is a lot riding on the outcome. Patients know that the success rate depends on how good the doctor is, which is why they have very high expectations from their doctor.
They will spend a lot of time hunting for Dr Right - an IVF expert who is approachable and affordable; who will design their treatment plan and personalize it specifically for them; and who will be available to answer their questions.

IVF Clinic in Mumbai - Parakh Hospital
When they go the clinic for their scans and monitoring, they want to be able to talk to their doctor, so he can check their treatment is progressing as expected, and that there are no unexpected surprises. They want a doctor who knows their name; who cares for them; and who is available to hold their hand and provide a shoulder to cry on when things don't go well.

It's very frustrating for patients to find that the doctor is never accessible. He is either travelling; or giving a lecture; or is away at a conference; or is treating patients at another clinic.

Even when he is in the clinic, he is so busy doing consultations with new patients, that he is not free to answer queries. When the patient is not sure if things are going well, or why the doctor has made a change to the protocol, they are forced to direct their queries either to the nurses or the assistants. The juniors are often clueless as to what is going on, because they are inexperienced or have been told that all patient's questions have to be answered only by the "Doctor" - they just have to carry out his orders !

When patients don't get a chance to get their queries clarified, they feel upset and abandoned.

This lack of information and transparency can become a major sore point for the patient. This is especially true when the cycle fails, and at this point the patient feels cheated. She is very likely to complain that she did not get good medical care, because the Doctor was not available.

Not only do patients need a lot of handholding - they expect their treatment to be personalized. When their care gets fragmented between the sonographer, the nurses, and the andrologist , they are understandably upset - especially when it seems that the left hand has no clue as to what the right hand is doing !

When the patient needs clarifications, the standard answer is - we are doing what the doctor has advised. Only he can answer your questions.

They would much rather have one person whom they could talk to - one person who was responsible for coordinating their care - and obviously they want this person to their doctor ! They want the doctor to be available to carry out all their procedures for them - including the scans, the egg collection and the embryo transfer. It's very frustrating when they find that they have to deal with a new doctor everytime they come to the clinic - someone who knows nothing about them and someone with whom they have no personal relationship.

They are always unsure if the new doctor knows what's happening - and it's very difficult to establish a rapport every time with a new doctor who happens to be on duty on that particular day. While these may seem to be relatively minor issues from a doctors point of view (because doctors are focused on providing high quality medical care), the fact still remains that IVF treatment is very highly emotionally charged, and IVF patients require a lot more attention and pampering as compared to the average patient !

Successful IVF clinics will develop processes to ensure sure that the patient's personal doctor is always available to take care of his patient throughout their entire IVF cycle. Not only does this ensure that the medical treatment is being carried out properly , it also allows him to provide the patient with continuity and emotional support.

He can provide a shoulder to cry on - or serve as a trusted sounding board , when patients are not sure what the next step should be, or if the cycle needs to be cancelled.

Patients also want documentation and openness about how the cycle is progressing. They need to know whether the follicles are growing well ; and if their treatment is progressing according to plan. When the protocol is changed, they need to know why this change was made - and what they can expect.

It's very frustrating for them when they have to wait for hours before they get a chance to talk to the Doctor.
The doctor should be accessible, so he can answer queries by email - or will ensure that calls are returned promptly by the end of the day. Patients want photos of their ultrasound scans and their embryos, so they have documentation that the treatment was carried our properly.

Before starting treatment, they need clarity about what the IVF treatment plan is going to ; how often they will need to visit the clinic; and how much it is going to cost them. They also need to know when they need to make payments - and what is included in the treatment cost, and what is not.

If there is an emergency or complication, they need to know whom to contact - and need to be sure there doctor is available to take care of them.

Finally, they expect a caring doctor, who will followup after the cycle, when the beta HCG test negative, so they know what their next action steps are, and don't feel abandoned just because they cycle failed.

All this requires a lot of coordination and care on the part of the doctor and a good IVF clinic will make sure that the doctor, who is the face of the clinic as far as the patient is concerned, is always available for his patient, when the patient wants and needs him.

Is IVF treatment Stressful?

The general perception seems to be that IVF is an extremely stressful treatment . Being infertile is stressful, so it seems logical to jump to the conclusion that going through Infertility Treatment in Mumbai India must be much more so - after all, so much rides on the outcome of the IVF treatment cycle !

However, an IVF treatment cycle can actually be a very positive experience . If patients follow certain guidelines, they are likely to have happy memories of the IVF treatment, irrespective of what the outcome is ! The trick is to have realistic expectations from the treatment.

Remember that IVF treatment gives infertile patients their best shot at getting pregnant, so that if you do an IVF cycle, you have peace of mind that you did your best so that you have no regrets later on in your life. Also, IVF treatment is elective treatment, which you take by choice - no one is forcing you to do this. Doing IVF allows you to exercise your own free will and decide - Yes, this is what I want to do, because it maximize my chances of having a baby. This can be an empowering experience, and you can look forward to it, because you're trying to control what is happening to you, rather than just being a passive victim of either a low sperm count or blocked tubes. In one sense, you're trying to write your own destiny, with the help of medical technology - what more can a human being do ?

Equally importantly, there is no medical risk involved in the IVF treatment in Mumbai India, when it's done properly. Unfortunately, there are still lots of myths and misconceptions that the hormone injections used in IVF make you fat; or increase the risk of breast cancer . You should do your homework, rather than listen to these old wife's tales. The IVF hormonal injections are natural hormones, which allow us to rescue the eggs which would have died anyway. They get excreted in the urine promptly, and do not have any long term consequences. Being armed with information therapy can provide a lot of positive energy when you're doing your treatment.

The biggest problem is that IVF patients often have not been counseled properly, as a result of which they often have very unrealistic expectations of success rates . Patients need to understand that the outcome of an IVF cycle is always uncertain, simply because human reproduction is an inefficient. enterprise - both in the bedroom, for fertile couples, and in the clinic, for infertile couples. No matter how good your doctor, once the embryo has been transferred into the uterus, we cannot control whether the embryo will implant or not. While it can be hard to come to terms with this uncertainty, once you factor this in, you realize that all that you (and your doctor) can do during the IVF treatment is your best - and the rest is out of your hands. This is exactly what the Gita advises - do your duty, without expecting any reward in return. This kind of philosophical approach towards IVF treatment will help to make the cycle itself much easier to cope with, so that it's easier to cope with the IVF emotional roller coaster ride.

One major advantage of IVF treatment is it helps you get in touch with other infertile couples . A major problem with being infertile is that you often feel isolated and ostracized . It seems that everyone else - office workers, friends, colleagues and relatives - is having babies. However, when you go to an IVF clinic, you get a chance to connect with lots of other infertile couples . It gives you a chance to network with them, and this provides invaluable emotional support . You realize that you are not crazy - and that there are many others who are in exactly the same boat you are in - you are not a freak.

Going through an IVF cycle can also create bonding between the husband and wife . It's when he sees his wife taking the IVF shots daily, that some husbands finally understand the depth of the emotional desire and the strength of the biological craving which a woman has, when she wants to have a baby. When he sees that his wife is willing to go through so much trouble in order to have a baby, he becomes much more empathetic, and has a much better understanding of her feelings - something which helps to make the marriage even stronger.

If :
  • You decide to do IVF treatment because you choose to do so of you own free will, for purely selfish reasons;
  • You have realistic expectations ; and
  • have been counseled properly,
  • You will find that an IVF treatment can actually be a very positive experience, rather than being a negative stressful one.

It's up to you ! If you take an active role, so that you are in control of your life, rather than thinking of yourself as a passive victim, you are likely to be happy with your IVF treatment - and getting pregnant is then just the icing on the cake !

Here is a quick way for solving female infertility through Tubal Cannulation Mumbai

Ratio of 1 in 4 women have difficulty getting pregnant due to blockage in fallopian tubes and Tubal cannulation is a procedure to help clear this blockage which is a common cause of female infertility.

Tubal cannulation may be done immediately after a procedure called hysterosalpingography or hysterosalpingogram. During this procedure, dye is first flushed through the catheter to identify and locate a fallopian tube blockage.

The doctor inserts a tube called a catheter that is guided over a wire. Ultrasound or real-time moving X-rays of the fallopian tubes may be used to guide your doctor to the correct area. The blocked area in the fallopian tube is opened up using a balloon on the catheter or with the wire.

Tubal cannulation can be done with or without anesthesia where you are given a mild sedative to relax and the procedure is less invasive than fallopian tube surgery and helps the doctor to better understand why the blockage occurred.

Usually it is an outpatient procedure which does not need a hospital stay and you can likely go home the same day. You can avoid more expensive and invasive surgical procedures.

Who should go for Tubal Cannulation ?

Once the sonography is done if the imaging test clearly shows a blockage in one or both fallopian tubes tubal cannulation is suggested. The cannulation procedure is most successful when the blockage occurs in the part of the tube closest to the womb (uterus), which is called a proximal tubal obstruction. It can be used for mid-tubal blockage also.

Some doctors recommend that women should consider tubal cannulation before having more expensive fertility procedures, such as in vitro fertilization (IVF).

Who should avoid Tubal Cannulation?

Tubal cannulation is not recommended for:
  • Patients having Genital tuberculosis and certain other infections
  • Who have undergone fallopian tube surgery before
  • If there is extensive scarring or damage in the fallopian tubes
  • Severe blockage that is difficult for a catheter to pass through
  • Distal blockage (in an area of the fallopian tube that is the farthest from the uterus)
Tubal cannulation may fail or may not work as well if you have:

  • A blockage in a part of the fallopian tube far away from the uterus
  • Certain blockages in the narrowest part of the fallopian tube, called the isthmus
  • Inflammatory condition of the fallopian tubes
  • Severe tubal disease or scarring
  • Tuberculosis
Risks of Tubal Cannulation

Tubal cannulation should only be done by a doctor who is well-trained in the procedure. Risks include: Failure to restore fallopian tube function or tearing or perforaton of the fallopian tube wall. Chances of a life threatening infection called peritonitis which occurs in the tissue covering the abdominal organs.

What can be expected after Tubal Cannulation

It is important to remember that successfully reopening the fallopian tubes does not always help a woman become pregnant as pregnancy rates depend on the:

  • Specific procedure performed
  • Location of the blockage
  • Cause of the blockage
Tubal cannulation helps restore fertility in many, but not all, women.

Women with severe fallopian tube disease who are not good candidates for tubal cannulation may wish to consider IVF and embryo transfer.

ICSI Treatment : Alternative Infertility Treatments Mumbai India

What is ICSI?

ICSI or intracytoplasmic sperm injection is used as a part of IVF treatment. For male infertility where there is a defect in sperm quality, intracytoplasmic sperm injection (ICSI) may be used, where a sperm cell is injected directly into the egg cell. This is used when sperm has difficulty penetrating the egg, and in these cases the partner's or a donor's sperm may be used.

Why ICSI ?

ICSI has been found to increase the success rates of IVF!
ICSI is also used when sperm numbers are very low!
Where there is a defect in sperm quality !
When there is poor sperm movement !
When the sperm is abnormally shaped !

When is ICSI done?

ICSI is done if regular IVF treatment cycles have not achieved fertilization.

It is done in cases of retrograde ejaculation where the sperm is retrieved from the man’s urine.

Sperm retrieval through TESE (Testicular Sperm Extraction) which is used when sperm is produced but not found in his ejaculate.

What is the procedure of ICSI?

ICSI is generally done as a part of IVF. Since ICSI is done in the lab, your IVF treatment won't seem much different than an IVF treatment without ICSI.

With regular In vitro fertilisation (IVF) process an egg is fertilized by sperm outside the body: in vitro. IVF is a major treatment for infertility when other methods of assisted reproductive technology have failed.

The process involves monitoring and stimulating a woman's ovulatory process, removing ovum or ova (egg or eggs) from the woman's ovaries and letting sperm fertilize them in a fluid medium in a laboratory.

If fertilization takes place, and the embryos are healthy, an embryo or two will be transferred to your uterus, via a catheter placed through the cervix, two to five days after the retrieval

Once the eggs are retrieved, an embryologist will place the eggs in a special culture, and using a microscope and tiny needle, a single sperm will be injected into an egg. This will be done for each egg retrieved.

How much does ICSI cost?

On top of the general IVF cost, ICSI typically costs between $ 1000 and $1500 which on average costs $12000 to $15000.

Is the baby safe in ICSI?
ICSI treatment carries a slightly increased risk of birth defects than normal pregnancy which carries a risk of 1.5 to 3%. Commonly found birth defects in ICSI includes Beckwith-Wiedemann syndrome, Angelman syndrome, hypospadias, and sex chromosome abnormalities.

There is some increased risk of a male baby having fertility problems in the future. This is because male infertility may be passed on genetically.

Chances of success rates by couples using ICSI with IVF compared to regular IVF is same once the fertilization happens. Even though ICSI procedure fertilizes 50 to 80% eggs just because a sperm is injected into an egg it does not guarantee fertilization will happen surely or sometimes the embryo may stop growing.

InVitro Fertilization today in India

What is Invitro Fertilization and who can go for it

In vitro fertilisation (IVF) is a process by which eggs are fertilised by sperms outside the body: in vitro in Latin means glass, because earlier biological experiments involving tissue cultivation outside the living organism was carried out in glass containers. That is how babies born with IVF procedures are also called test tube babies. IVF is a major treatment for infertility when other methods of assisted reproductive technology have failed. The process involves monitoring and stimulating a woman's ovulatory process, removing a healthy ovum or ova (egg or eggs) from the woman's ovaries and letting sperm fertilise them in a fluid medium in a laboratory. The fertilised egg (zygote) cultured for 2–6 days in a growth medium and is then transferred to the patient's uterus with the intention of establishing a successful pregnancy.

Females who have problems in the fallopian tube and men who have defect in sperm quality are suggested to go for the IVF treatment.

The method follows:

in vitro fertilisation could be performed by collecting the contents from a woman's fallopian tubes or uterus after natural ovulation, mixing it with semen, and reinserting into the uterus. But additional techniques are used for successful pregnancy like ovarian hyperstimulation to retrieve multiple eggs through ultrasound guided transvaginal egg retrieval, egg and sperm preparation, culture and resultant embryo transfer into the uterus. IVF can be performed by collecting a naturally selected egg from woman’s natural menstrual cycle without the use of any drugs. The next step-up method which is called modified natural cycle IVF, medication is used for 2–4 days during woman’s natural cycle to avoid spontaneous ovulation and to make the treatment more successful. Semen is prepared for fertilisation by removing inactive cells and seminal fluid in a process called sperm washing and if it is provided by a sperm donor, it is usually prepared for treatment before being frozen.

The sperm and the egg are incubated together at a ratio of about 75,000:1 in a culture media as per studies in 2013 that a duration of 1 to 4 hours results in higher pregnancy rates than 16 ot 24 . intracytoplasmic sperm injection (ICSI) process is undertaken where a single sperm is injected into the egg in situations where it is low sperm count.

IVF success rates are the percentage of all IVF procedures which result in a favorable outcome.

About 10-12 percent of Indians have trouble conceiving a child. But several myths keep couples from seeking help

Some Myths vs Facts about IVF and Pregnancy: Courtesy Dr Firuza Parekh (The Complete Guide to Becoming Pregnant)

Myth
Eating spicy food causes ovulation, low sperm count, miscarriage, and premature delivery.
Fact
Eating spicy food only occasionally disturbs the gastrointestinal system and sometimes causes heartburn.
Myth
Seven to eight days of abstinence are required before a semen analysis, or before giving a sample for IVF/ ICSI/ sperm freezing.
Fact
Increased days of abstinence can, in fact, decrease the sperm motility. Two or four days of abstinence are adequate for a good semen sample.
Myth
Masturbation causes impotency and low sperm count.
Fact
Masturbation should not be associated with guilt. It does not impact fertility.
Myth
The chances of pregnancy are reduced if the ejaculated semen comes out of the vagina after intercourse.
Fact
Ejaculated semen liquefies after a few minutes in the vagina. Some seminal fluid is bound to come out. This does not decrease the chances of pregnancy.
Myth
The sexual position determines the gender of the baby.
Fact
No sexual position has any effect on the gender of the baby. There may be other compelling reasons though, for some sexual variation.
Myth
Having sex daily increases the chances of pregnancy.
Fact
Daily sex does not increase the chances of pregnancy and may, in fact, decrease the sperm count on the day of ovulation. In men with low sperm counts, this may be detrimental to the odds of pregnancy.
Myth
PCOS (polycystic ovary syn drome) goes away after having a baby.
Fact
PCOS is a condition that has a genetic basis. It does not go away by itself. Treatment needs to be continued.
